Terms and Conditions for Diagnostic Assessment
 

Version: August 2025

This agreement sets out the terms under which we will work together. Please read carefully before signing.

Services Offered

I provide assessments for adults (18+) in the following areas:

  • Autism

  • Learning Disability

  • Mental Capacity

Each assessment includes:

  • Interviews with you (and where relevant, family/carers)

  • Standardised assessment tools and questionnaires

  • Feedback session explaining results and recommendations

  • A detailed written report

  • Optional follow-up sessions if further support or clarification is needed

Fees & Duration

  • Initial telephone consultation ~30 mins - Free

  • Screening Discussion + Consultation~60 mins - £225

  • Autism Assessment ~ 90–120 mins - £1,075

  • Autism Follow-Up Session ~ 60–90 mins - £225

  • Learning Disability Assessment ~ 150–180 mins - £1,775

  • Mental Capacity Assessment ~ 120–180 mins - £1,475

  • Fees are payable in advance of the assessment. Once paid, fees are non-refundable, although sessions can be rearranged, minus £100 for admin, room rebooking and rearranging. 

Preparation and Attendance

Screening

  • Book a Screening Consultation (for Autism, Learning Disability or Mental Capacity assessments).

  • Complete the screening forms sent by email (a relative, partner, or friend may help).

  • Attend a screening discussion, where we answer your questions, having read your returned forms, and provisionally schedule your assessment.

Arranging the Diagnostic Assessment

  • Complete the intake form, terms and conditions, and consent forms (including consent to contact a relative/friend/partner).

  • Pay the full fee to confirm your booking (fees are outlined in service information).

  • You will then receive confirmation of dates and times.

  • Developmental and background questionnaires will be sent to you and a relative/friend. These take 1–3 hours to complete and must be returned at least 14 days before your first assessment session. If not returned, your assessment will need to be rearranged. If completing them is difficult, we can arrange an extra session to support you.

Attending the Assessment

  • Assessments usually last around 2 hours and may be attended alone or with a relative/friend.

  • Breaks, snacks, stretching, and use of fidget items are encouraged as needed.

  • Please let me know about any adaptations that would help you.

  • Following the assessment, you will receive a detailed report.

Payment

  • Invoices are issued to you or your insurer (if agreed).

  • Payments are strictly non-refundable once made.

  • If self-funding, please pay using the secure link provided in your invoice.

  • Insurance-funded assessments will be invoiced directly to the insurer (where agreed).

Responsibility for Fees and Insurance

  • You are responsible for all fees. If your insurer does not cover them, the balance must be paid by you.

  • Some insurers require an excess payment; this must be settled within 30 days of starting treatment.

  • An authorisation code is required before treatment begins if billing to insurance.

Cancellations

  • 72 hours’ notice is required to cancel or reschedule. Late cancellations are charged in full.

  • A new appointment will need to be booked and paid for.

  • Insurance companies generally do not cover late cancellations or non-attendance, so these costs fall to you.

Confidentiality and Data Protection

Your sessions are private, except in the following circumstances:

  • If there is a significant risk of harm to yourself or others.

  • In a medical or psychiatric emergency.

  • If you disclose concerns about a child’s safety or welfare.

  • If you disclose serious criminal activity (legal reporting is required).

Notes are taken electronically during the session and are uploaded securely and encrypted. Records are retained for seven years after treatment and then deleted.

Health insurers may request access to records for case management or quality assurance. Courts may require access to notes, which I must provide if ordered.
